How do I make an appointment? If you are new to my practice, let's start with a free 15 minute phone consultation so I can get a better understanding of what you are looking for. Please use the contact form to tell me a little about yourself and when you are free to talk. Existing patients may request an appointment using the contact form or calling me at 619.393.9573.
Do you accept insurance? I do not accept insurance nor do I work directly with insurance companies. However, if you have a PPO insurance plan you may be able to get a portion of my fees reimbursed from your insurance company. Upon request, I can provide a receipt/Superbill for you to submit to your insurance carrier, which may reimburse some of the fee.
What type of payment do you accept? Payment by cash, check or Venmo is due at each appointment unless other arrangements have been made in advance.
I'm a new patient. What should I bring to my initial appointment? Please email all required formsandpertinent medical records to firstname.lastname@example.org to your appointment. Please also bring your preferred form of payment to your first appointment.
What is your cancellation policy? Appointments canceled or rescheduled with less than 24 hours notice will be charged a 50% fee. Missed appointments will be charged 100% of the appointment fee. There is no charge if an appointment is canceled or rescheduled with more than 24 hours notice.
Where do I park? Our office is located in Hacienda Del Mar. There is free parking adjacent to the office building.
